Mrs Anna Bungay Account no 10177720 Sort Code 07 08 06 Ref- Childs name 1 Session=£20 TERMS & CONDITIONS AND INFO Uniform and Requirements Appropriate clothing suitable for dancing must be worn including socks and trainers. (leggings/joggers recommended) Mini Dance Performers T-shirts and merchandise will be available for purchase during the event but are not compulsory to buy. It is essential you provide your child with their own labelled water bottle and packed snacks (No nuts please!) We have an awesome dance team with plenty of staff/helpers to support and make sure they have lots of fun! Safety and liability Myself and Mrs A Gaynor are both DBS checked, fully safe guarding qualified and First Aid trained. I am a fully qualified and experienced Dance Teacher. Anna Berryman Dip.RBS(TC) ARAD,AISTD MDPs does NOT accept any responsibility for loss/damage of personal property. Or any injuries arising from errors of the child. MDPs holds Public Liability Insurance. COVID Policy Hand sanitizer is available at all times. Use of the toilet is available. PPE will be worn in the case of dealing with an incident. Please inform me immediately if your child has a temperature or any symptoms of Covid19. Photographs and video footage of pupils attending MDP may be used on our website/facebook page for publicity purposes. If you do not wish for your child’s image to be used for these purposes, you must inform me in writing ASAP. Pick up/drop offs This will take place at St Marys Church hall,Church Walk, Walton on Thames, KT12 2QS. Collection can only be by the Parent/Guardian. Pls inform me if someone else is collecting your child. Registration starts from 10am and pick up is at 11.30am...pls be patient with both of these to allow me to keep the children safe.
